Podcast Guest Consent & Release Agreement

By completing and submitting this questionnaire, you (“Guest”) acknowledge that you have read, understand, and agree to the following terms:

1. Consent to Record and Publish

I voluntarily consent to being recorded (audio and/or video) for a podcast interview with Beth Hovis. I grant permission for the recording to be edited, produced, published, distributed, and shared in whole or in part, in any format or medium, including but not limited to podcast platforms, video platforms, websites, email communications, and social media, now and in the future.

2. Ownership and Rights

I understand that the podcast host/producer, Beth Hovis, retains full ownership of the final recorded episode and any derivative works (including edited clips, transcripts, quotes, and promotional materials). I retain ownership of my original ideas and intellectual property, but do not claim ownership of the recorded interview or its edited versions.

3. Editing and Removal

I acknowledge that editing decisions are at the sole discretion of the podcast host/producer. I understand that once an episode is published, there is no obligation to remove or alter the content at my request.

4. Guest Representations

I affirm that:

• The information I provide and the statements I make during the interview are truthful to the best of my knowledge.

• I am not violating any confidentiality agreements, copyrights, trademarks, or other third-party rights.

• Any personal, professional, or educational opinions I share are my own.

5. No Professional Advice

I understand that the podcast content is shared for educational and informational purposes only. Nothing I say, nor anything discussed during the interview, is intended to constitute medical, legal, financial, or other professional advice. Listeners are encouraged to seek appropriate professional guidance for their individual circumstances.

6. Compensation

I acknowledge that my participation is voluntary and that I will not receive monetary compensation for my appearance on the podcast. I understand that exposure, audience reach, or promotional outcomes are not guaranteed.

7. Name, Image, and Likeness Release

I grant permission to use my name, voice, image, likeness, business name, and biographical information for purposes related to the podcast, including promotion, marketing, and archival use.

8. Release of Liability

I release and hold harmless the podcast host, producer, and affiliated entities from any claims, demands, or liabilities arising from my participation in the podcast or the use of the recorded content.

9. Governing Law

This agreement shall be governed by and construed in accordance with the laws of the State of Georgia.
